FIRST TIME CLIENTS – PLEASE READ
In order for us to spend quality time with all our clients and due to the fact that availability is limited, we require that first time clients purchase a time-slot on our web store or use the product link at the bottom of this form.
First time clients – you will need your Order Number in order to fill out the appointment form below!
Your one time $50 time-slot charge is applied towards your service at the time of your appointment.
If you book online we will be contacting you to confirm your appointment and to secure your booking-fees-for-First-Time-Clients appointment time purchase.
Please make sure you create a client profile on our web store so we are able to contact you regarding the appointment.
Groupon and Gift certificate clients Do not have to purchase a time-slot but the following rules apply:
Enter the voucher or Gift certificate number for groupon clients please enter the voucher number below the barcode.
Vouchers and Gift certificates not re-booked or canceled within 24 hours may be forfeited. See the cancellation policy below for more information.
Note: Returning clients do not need to purchase a time slot in advance.
If we receive less than 24 hours notice to cancel or change the appointment, a cancellation fee will be incurred in an amount equaling the time slot purchase.
Vouchers/Deposits not re-booked or canceled with 12 hours or less notice are forfeited
